The deep plane facelift is an advanced facial rejuvenation surgery that repositions deeper structures of the face that have descended with age. Because it goes beneath the superficial layers of the face, this technique requires detailed knowledge of facial anatomy.
At Dilworth Facial Plastic Surgery, dual board-certified, fellowship-trained facial plastic surgeons Dr. Andrea Garcia and Dr. Joshua Surowitz perform every deep plane facelift together. Both surgeons trained under recognized experts in the deep plane technique and have performed more than 600 deep plane facelift procedures as a team.
A deep plane facelift is an advanced facelift technique that addresses moderate to significant sagging in the cheeks, jowls, and neck that develops as underlying tissues lose support and descend over time. It repositions the deeper facial structures to their original youthful position, along with the overlying skin, for natural-looking results that don’t look tight or pulled.
“The nice thing about a deep plane facelift is you’re actually anatomically suspending things from where they came. We do not stretch or pull the tissue to where it never was; we resuspend it as it used to be. That’s why patients look like themselves earlier, versus looking like somebody totally different.”

A deep plane facelift, which typically includes a deep neck lift, provides comprehensive rejuvenation of the midface, lower face, jawline, and neck by:
Because facial aging involves both tissue descent and volume loss, Dr. Garcia and Dr. Surowitz include facial fat grafting with every deep plane facelift for a more comprehensive rejuvenation by restoring volume in the temples, midface, under-eye hollows, and pre-jowl area.
Dilworth Facial Plastic Surgery combines the expertise of two dual board-certified, fellowship-trained facial plastic surgeons for unparalleled patient care. Dr. Andrea Garcia and Dr. Josh Surowitz offer a unique collaborative approach to facial plastic surgery, working as a compassionate and highly trained duo to bring out the very best in you.
Credentials You Can Trust

You may be a candidate if you are in good overall health and have noticeable facial sagging, jowls, or loss of jawline and neck definition. There is no single “right” age for a facelift; our surgeons consider your anatomy, degree of facial aging, health, and cosmetic goals instead. During your consultation, both surgeons evaluate you and determine which facelift approach is most appropriate for the concerns you want to address.
Our surgeons perform deep plane facelifts in our fully accredited, on-site surgical suite with general anesthesia administered by an M.D. anesthesiologist. Dr. Garcia and Dr. Surowitz carefully plan the incisions around each patient’s anatomy, placing them within the hairline and natural creases around and behind the ears to help keep the resulting scars discreet.
Through these incisions, the surgeons work in the deep plane beneath the SMAS, a supportive layer of facial muscle and connective tissue. They release retaining ligaments that anchor the facial tissues to deeper structures and reposition the SMAS and overlying skin and soft tissue together, closer to their original, more youthful position. The deeper tissues then provide support for the lift rather than placing tension on the skin.
Dr. Garcia and Dr. Surowitz operate together throughout the procedure. When combining deep plane facelifts with other facial procedures, their two-surgeon approach allows surgery to be completed more efficiently and with less total anesthesia time than a single surgeon working alone.
Most patients return to work and regular routines about 3 to 6 weeks after a deep plane facelift, which is slightly longer than after a traditional facelift. Swelling and bruising are typically most noticeable during the first 1 to 2 weeks.
Dr. Garcia and Dr. Surowitz provide detailed recovery instructions before surgery and closely monitor your progress afterward. You also have direct access to both surgeons throughout your recovery. Read our blog post for details about deep plane facelift healing and scar care.
Deep plane facelift results are long-lasting, at least 10 to 12 years, because the deeper facial tissues are repositioned and supported. Keep in mind that your face will continue to age naturally, but from a more youthful starting point.
Dr. Garcia and Dr. Surowitz bring specialized training and extensive shared experience to deep plane facelift surgery:
With this collaborative approach, our surgeons bring their experience and unique perspective to creating natural-looking results tailored to each patient.
